At the time he was really really sick, and so I didn't really think I'd ever be doing anything with it. more of a personal record. not quite sure why I've stitched it together today. maybe because he has come such a long way and it's now bearable to think about how sick he was back then. maybe because every day we are getting closer to him coming home. I bought a crib yesterday. knowing i won't be going to do this ever again, I though I'd get the nice one I really wanted last time, but didn't. why not, eh? you only have your last child once.

Day 59 - no news is good news

just a quick update to say everything is FINE.

Nothing is happening, really. After Henrik was moved OUT of HDU (see my last post) he got some kind of bug, bit like a cold i reckon, and went back INTO HDU after a few days. It was ok, I wasn't too disappointed, though psychologically you really don't want to be moving backwards I suppose. Since then, he has been on between 0.05 and 0.02 liters of oxygen, which is very little, but addictive! HE is breastfeeding once a day and tube fed for the rest of his feeds (though they also top him up after breastfeeding, as he gets very tired quickly). he has occasional brady cardias (drop in heart rate) which is scary but normal, and yesterday he had his first bath! He is starting to wake up for feeds and gets really angry when he is hungry.

we are having a discharge meeting next week to plan him coming home on oxygen, but no news yet really...

Day 52 - Out of HDU

This morning, Henrik was moved out of the High Dependency Unit and into one of the smaller nurseries. he is not in the main nursery (last stop before home) because he was kicked out of HDU mainly because they needed the space and he'd been off Cpap for 24 hours.

he is looking a little pale today, but his sats are good and he worked really hard and did very well at the breast. The consultant also told me they are setting up a meeting with the community nurses to arrange for him to come home on oxygen. this will take approximately 2 weeks and then it's up to his feeding as to when he can go home....

cross your fingers it'll be before christmas...

Day 50 - off cpap completely....

Henrik took yet another exciting step this morning.

When I arrived at around 9am, he was FURIOUS. I kid you not, his face was bright red and he was crying (as in what babies do when they're upset, but which Henrik hasn't done yet...) The nurse had just put his Cpap back on and he was having none of it. SHe gave up in the end (a distressed baby with crazy heart rate and low saturations is much worse than one with slightly low oxygen intake) and put him on low flow prongs instead. he settled immediately. strange to see him want something, or, rather, NOT want something...

when the consultants came around, she agreed we'd try him without cpap altogether, as he hates it so much. he isn't being moved out of HDU just yet, he will need to prove that he can cope, but it basically means he is doing all his own breathing and not receiving any help bar a little bit of oxygen. and so far - 6 hours later - he is doing really well. he even had another go at breastfeeding this afternoon, and did really well. then he fell asleep :)

oh,and i caught up with the consultant in the corridor, who said that she coudn't' see any reason why he shouldn't be home for christmas... but that's still 7 weeks away and as we know, a lot can happen in 7 weeks...

Day 48 - many many baby steps

sorry for lack of updates. we had visitors all weekend (hi lisa & wiz!) which was really nice, but meant I had little time for blogging.

anyway... SO much has happened!

on friday onwards, Henrik has bravely attempted to feed himself! with much enthusiasm and a bit less luck, he has latched on, as it is so charmingly called, to my breast and is trying really hard to eat. he gets really tired really quickly, mainly because he is SIMULTANEOUSLY trying his luck off cpap, so we have only had moderate success, but i am not complaining. everything has to be learned, including sucking. and he seems really good at it. or maybe he is just a *boy* ;-)

anyway, he gained a whooping 105 grams over the past 4 days, so he is now 1570grams, which is pretty incredible, and also means he is getting too big (yes i said those words) for the incubator! this morning, after slow-cooking all weekend (confit?) they decided today to move him into an open cot. yes, the ones you can just PICK your baby out of, and the ones you can TOUCH him without opening little windows. it's really great news.

he didn't cope tremendously straight away, and as his temperature rose, the number of blankets covering him increased (5 at one point, plus baby-grow and hat!) but once they move forward with a premmie (and this counts for everything from ventilation and cpap to incubators) they like to move fast, push them along and try their VERY best not to move them backwards. in other words, they really dont want to put him back into the incubator unless they really really have to. so instead they added more blankets, and increased the temperature in the room to tropical levels (all the other babies are now cooking and the staff and visitors sweating.... ;-)... )..

and he is fine. of course on this momentous day I forgot my camera at home, but I will get some tomorrow (did i make a similar promise before??? ;-))

Day 43 - photo-wednesday!

please head over to flickR for some photos of Henrik...

he's up to 1465 grams today and they are trying him off cpap now and then. he came out for a cuddle and as he was hungry he was really trying to move across me to well, where the food is, which was very encouraging. he then fell asleep. now i smell of incubator!

Day 42 - bye bye intensive care

In the early hours of this morning, Henrik moved from the Intensive care unit (ICU) to the high dependency unit (HDU). This is literally across the corridor, but in terms of how status it is a massive move. He is still on cpap, he is still in about 30% Oxygen and he is still quite unreliable in terms of dropping his heart rate and saturations, but - BUT - it's a massive psychological move for everyone, he no longer has one nurse allocated to him at all times, and when i popped in this morning, WITH Helene, the first thing they asked was if i wanted him out for a cuddle. alrighty then.

so we're moving up in the world, henrik and the rest of us, so expect lots more photos (probably on flickr), well, as soon as i get them off the camera and stuff. :)

very good day, indeed.

Day 40 - experiments without cpap

oh yes things move fast in super-preemie land!

this morning, during his nappy change, the nurse took Henrik off his cpap (he had a funnel of oxygen but still!) and he coped fairly well. when we came in i was all, "ooh so lovely, wish i could have seen him, haven't seen him without anything all over his face yet" and the nurse felt bad for me and TOOK IT OFF AGAIN! :) he coped marvellously, even gave me a smile (ok, he had wind, but who cares, it LOOKED like a smile!) - she even took some photos for me, but i haven't scanned them in yet, sorry!

He has actually got hair! i was shocked. and he looks a lot like Helene did, once i get the photos into digital form, I'll post one of Helene too to compare.

and i don't need to say that it really made my day! :)

Day 38 - off advanced, onto conventional cpap

ok, so the subject says it all really. my updates from now on are probably going to be quite boring. no more intense medical dramas and - hopefully - no great emergencies (though we still have the great beast known as R(etinopathy) O(f) P(prematurity) to come...) - but more cheerful hopefully...

Henrik's now on 2 hourly feeds, a whopping 19 mls every two hours, which will fatten him up a bit. Spoke with one of the consultants who also said they might try and get him off the ventilator today or tomorrow or... how long is a piece of string..?

but he's fairly settled, still recovering from the surgery, which, in case i forgot to mention it, went really well and he coped with magnificently. His heart rate kept dropping this morning, which i am told could be his body readjusting to life post-duct, but still freaks me out. his saturations are better, mostly, though, so i should stop being so sensitive and appreciate the fact he is on minimal oxygen, with really low settings. I think I might be having a post-op reaction as well, so the fact i am spending the afternoon at Helene's school drawing 8 legged cats (she hasn't quite worked out the anatomy of cats yet) with Helene is probably a blessing in disguise.

+++++ UPDATE ++++++

I just spoke to sarah (H's nurse) and she told me they've taken the chest drain out AND taken him off the ventilator! :) He is in quite a lot of oxygen (80%!) but he is OFF the ventilator. with that amount of O2 he might have to go back on, but they will hold off as long as possible which means he'll really get to exercise those lungs! yay for henrik and yay for cpap! :)

PS. Raquel, he struggles hard enough to open his eyes at the moment, let alone keep them open long enough to photograph him.. I shall try, but no promises... they have stopped his morphine this evening as well, so that might wake him up a little. will bring camera in tomorrow... :)

Day 32 - post-op

Henrik's resting. he is on morphine, and on meds for low blood pressure. the doctors can't hear a murmur anymore, which means the duct has probably closed properly. he is back on feeds, 3 ml pr hour. despite everything, he looked well this morning. he is such a trooper.

no other news, really. just relaxing and getting over it. me, that is :)

Day 31 - surgery - UPDATE!!!

Henrik's off to Bristol today for his PDA ligation. There are no beds, so they're taking him up, operating, and then taking him back home tonight. it means there's no point in me going up, so David's taken the day off and spending it with me at home. I am all brave on the outside and all worried on the inside.

we need to give our consent over the phone, and the consultant is keeping up updated via his mobile through the day. unsurprisingly, everything is delayed, so they won't be home till midnight, which means I won't see him post-surgery. not till tomorrow.

wish him luck.

+++++++UPDATE++++++++

just had a call from the consultant - surgery went well and they're on their back from Bristol. (at the bottom)

today, in between

* trying to work out where Henrik's going to have his pda ligation
* changing his bedclothes as he wee'd all over them whilst i changed his nappy (boys!)
* trying to negotiate Helene's 4 year ex-prem developmental check
* and attending a lecture on virtual life at the university

I had my first cuddle with Henrik!!

it was lovely. he was really good, on a mere 25% oxygen (until the consultant arrived, when he decided to start de-saturating and it had to be put up to 34%) and even opened his eyes for me (towards the end, after sleeping through most of the fun)

it's mad though, with the ventilator and the leads and all the necessary blankets to keep him warm, me sitting uncomfortably terrified of either dropping him (he's still relatively small at 1.45kg) or *doing something wrong* or something just *happening*, the poor nurse hanging around my neck holding the ventilator in the right place, and Henrik just sleeping through it.

on top of everything, the consultant decided to give me a detailed update WHILST I was holding him (read: panicking) so most of the information came in one ear and out the other.

but i *think* his ligation is becoming increasingly urgent as he's got some liquid in his lungs, and they are now looking at sending him to Birmingham (!) as there's no cots in Bristol or Cardiff. There's no cots in Birmingham EITHER, but this is somehow not an issue...

anyway, the cuddle was lovely especially as he coped so well and was so peaceful and happy throughout! :)

+++++++++ UPDATE++++++++++

Just spoke to the nurse, who tells me that there are no beds ANYWHERE and probably won't be for a week. and we've got an infection free baby, but god only knows how long that will last, so the consultant is obviously keen to get him fixed as soon as possible... SO... the plan NOW is to do a drive-through to either Bristol OR BIrmingham, on Friday, DESPITE the distance. the consultant, a registrar and a nurse will go up with him in the ambulance. so complicated in the world of the neo-nates...

<link rel='alternate' type='text/html' href='http://henrikisak.blogspot.com/2007/10/day-29-cuddles.html' title='Day 29 - cuddles - UPDATED!!! I'll update when i know. if i don't, I've probably had to rush off...

another thing, *little* Henrik is actually in the 50th percentile! they weighed him last night and he is 1450grams, which is HUGE really especially considering how sick he's been. Henrik's un-weanable. he is simply not coping with lower pressures, and will definitely need the pda ligation. i don't know why they bother telling me, i knew it already. now i just want it done, so we can get him off the ventilator and onto cpap SO I CAN HAVE A CUDDLE! yes, I still haven't held my son, and it's starting to take its toll. maybe it would be easier if he didn't look so damn cute and cuddleable, there, underneath all the tubes, secretions and lines.

and he copes so well! i mean, the boy's had sepsis, rsv and chest infections, he has an open pda and he is *only * on 36% oxygen! ok so his pressures and up and down, and he desaturates if you look hard at him, but he self-corrects pretty much consistently, and looks nice and pick even through the most severe de-sats.

so i am rather proud of him. they say boys do worse than girls, and ok, he's caught infections Helene never had, but still. underneath, I think we have another fighter on our hands. and thank for god for that!

Day 25 - consultants and notes...

we all went in to see Henrik this morning, me because I always do, Helene because she was spending the day with me and David to see if he could sneak a peak at Henrik's notes.

<digression> the 'reason' we're not supposed to see his notes are somewhat blurry to me. the nurse i talked to said they were intended as "communication between the medical staff" and that if requested IN WRITING, parents would be allowed to see them. I imagine they don't let parents see them because they are stuffed full of information, information which it is pretty difficult to both digest and understand. there are medical terms, conditions, diagnoses, scans, medicine charts, etc etc - and presumably most parents would get somewhat overwhelmed by all this (i still do and ive been through it once already!) </digression>

amusingly, with his stern looks and "don't mess with me" demeanour, David wasn't once asked not to read Henrik's notes (bloody patriarchy!) by the nurse (ok it wasn't the same nurse who 'told me off' bit still...). He took his time too, and even picked up a couple fo things we hadn't been told - like the fact they were investigating whether Henrik's got cysts on his brain - he probably doens't - and even if he had they'd be in a non-dangerous position - but still... it's nice to know, right?

Following his detective work, we talked to the consultant, the lovely Mr Morris (who reminds me SO MUCH of Helene's lovely consultant Mr Amess), who talked us through the plan for treating Henrik's PDA. They still can't send him to Bristol for the ligation (he still has RSV) but once he is well, they will most likely send him off. At the moment there are no cots free though, so they might have to do a drive through - which means they'll ambulance him over, the surgeons will operate ON THE UNIT in Bristol and then they'll drive him home again, a procedure that takes max 12 hours. mad.

Thinking about our 2 day trip to London when Helene had her PDA ligation, staying at St Thomas's and negotiating our way through London to Guy's, the first evening (before the operation) with all our friends, sitting by the themes, feeling totally blurry and surreal in the head, and comparing that to not even GOING to Bristol because it's such a quick "there and back" procedure, is really strange...

anyway, after all that David had a private lesson with the consultant about, among other things, how to read scans and x-rays, how to PERFORM scans on neo-natals and general information about care of premature babies, while I took Helene to play in the waiting room. only david...

Day 23 - eyes and ducts

Henrik looks mottled and pale, but seems better. his ventilation is
yo-yoing. frequency is up to 50 though his O2 is between 30-40 and
pressures down to 16 over 6, which is good. but duct means he de-sats
a lot. same old same old. Henrik's oxygen levels are really good, 25%, his pressures are right down to 16 and his rates lie around 30 (which they tell me is good) - HOWEVER, his duct seems to have opened again, he is pale, and they think he might have a chest infection. At least he is tolerating his feeds (a whopping 7.5 ml/ hour) and going to the toilet regularly.

I popped in this morning to see him, but as I am not totally well myself, I didn't want to stay long (not just for his sake and mine, but for all the other parents. I know if I was in and some mother was coughing around the unit I'd be quite unhappy.). So I am back home, and trying to rest. I popped in very quickly earlier (i have a cold/cough and am terrified of passing it to him) and in an effort so far reserved for his older sister, he opened both his eyes WIDE and i could swear he was looking for me from where the sound of my voice was coming. He has attempted to open them before, but has either been too sick, too drugged (morphine) or they've been too puffy with odema and so the effort was too much for him. So when he opened his big, very dark eyes I could hardly contain my excitement. Turns out they've given him caffein to give him a kick (they're trying to wean him off the ventilator) but still... much nicer to have an alert, responsive premature baby than a lethargic, sleepy one.

summary... so far... (17 days)

=== just read through this and realise it's rather badly written and also quite messily narrated, but I don't have the energy to tidy it up right now... it's meant as an update for those who are interested, not as an entertaining blog to secure me a book deal :) ===

having a baby in special care is confusing. things move so fast sometimes, and sometimes they swing from "doing really well" to "doing really badly" overnight, only to swing back again the next morning. there are so many hoops to jump through, so many milestones that need to be met, and fast. as a parent you tend only to remember the things they *didn't* do on time, mainly because it meant lots of worrying, days and nights of waiting, followed by either extreme relief when the thing that had to be done (like pooping for the first time!) has been done, or the agony or medical or, sometimes, surgical, treatment (like the PDA).

Henrik's journey over the past two and a half weeks has been no different. I am already struggling to remember what happened when, hence the attempt to start logging things in here.

day 1 or 2 he was treated for Jaundice.
day 3 or 4 he was treated with Ibuprofen for PDA
(I can't even remember how many blood transfusions he's had to date)

I know that when he was 7 days old he hadn't been to the toilet yet, and the doctors started telling us about NEC, or Necrotizing Enterocolitis, which is a pretty nasty infection. Three days later, everything seemed to go pearshaped. His oxygen dropped to 100% (and like the nurse said, "there's not many places to go from there"), his pressures were up, he was white as a sheet and his blood gases were rising. his CRT (which measures infection in the body) jumped from 19 to 93 to 190 and the duct, which had "pretty much closed" after the treatment the week before, had opened again. not a great sign..

Turns out he had sepsis, which caused all kinds of concerns amongst the doctors (I was sitting outside sobbing at this point), and basically meant that he was taking about 5 steps backwards. The only good thing about this was that, despite not having pooped, this probably meant they could rule out NEC (which was correct, he never had it). the bad news was he was really really sick, and struggling to breathe. The 100% oxygen also meant they moved him from the 'normal' ventilator to the high frequency oscillator which, ok, looks worse than it is, but still freaks the hell out of me. unconscious, blown up, vibrating baby is not a pleasant sight. they gave him 3 different anti-biotics (they also suspected Candida and he was on anti-fungal anti-biotics for at least 6 days), morphine and had to cut right back on the EBM (in fact, i think they stopped it altogether, which is ironic, as it helps to fight infections and the TNP they give instead increase risk of infection... but there you go...)

anwyay, i think that's about all the problems he's had so far. Since last friday (a week ago today), he seems to have overcome the infection, and another course of ibuprofen has "pretty much closed" the duct for the second time and he ha gradually worked up his feeds to 6 ml an hour (!) - this is full feeds for his weight, and they've stopped his TPN and taken the long line out.

goals for next week: gaining more weight (he has already gone up from 970grams to 1250 grams since birth!) and weaning him off the ventilator... fingers crossed...

<link rel='alternate' type='text/html' href='http://henrikisak.blogspot.com/2007/10/summary-so-far-17-days.html' title='summary... so far...
